feat: use new hetzner cloud instance group abstraction

Rewrite the plugin framework to use the Hetzner Cloud instance group abstraction.

• Create new instancegroup abstraction
• Allow passing static credentials
• Split logic into more layers
• Remove docker-machine-driver abstraction
• Include server type in instance group ID
